Starmate 5 $50 Rebate Requirements

To Qualify for an $50 Mail In Rebate when purchasing a Starmate 5 player

1. Purchase a brand new Starmate 5 from TSS-Radio.com between August 30th, 2009 to September 26th, 2009.
   Note: Subscription to Sirius Radio sold separately.
2. Mail in the following items to the address specified below:
   • A completed rebate form (inclouding Radio ID and your signature).
   • A copy of the sales receipt for the Starmate 5 showing the purchase date was between August 30th, 2009 to September 26th, 2009.
   • The original Radio ID cut from the inside hard plastic packaging and included with the sales receipt and completed rebate form.
3. All rebate documents must be postmarked no later than October 26, 2009 to be eligible for this rebate. Please make a copy of all rebate documents for your records.
4. Please mail all rebate documents to the following address:
